‘Pass it On’ is a project that uses human-centred design principles to facilitate conversations about death. The pack of conversation prompt cards guide questions to ask about end-of-life planning between family members. The first step to end-of-life plan is talking about it, by asking the questions on the cards people will be better prepared for the human experience everyone will go through.
